New Patients
Becoming a Cornerstone Family Practice patient is easy! Just follow
these simple steps:
• If you're currently enrolled in an HMO insurance plan, call the
customer service number on the back of your insurance ID card and tell
them you wish to change your primary care provider (PCP).
• Print out and complete our Transfer
of Medical Records form and fax it to us. Or, if you prefer,
call our office at 978.948.3902 to have the form faxed or mailed to you.
After you've completed and signed the form, forward it to your current
PCP so that they can send us your records.
