Appointments (non-urgent)

If you'd like to schedule an appointment for a non-urgent matter,

• Step 1: State your name, address and telephone number

• Step 2: State the reason you need to have an appointment.

• Step 3: State the date and time you'd like to be seen and the Clinician with whom you'd like us to schedule the appointment.

We will respond within 72 business hours. For any urgent matters, please call our business office at

Referrals

If you need a referral, Email -- referrals@cornerstone-ma.com seven business days prior to the scheduled visit with the specialist.

• Step 1: State your name, address and telephone number as well as the name of your Insurance carrier, and your identification number.

• Step 2: State the name of the specialist, his/her specialty, phone number and address.

• Step 3: State the reason you are going to the specialist and the date of the Appointment.

Some HMO's are more restrictive than others, so if you have a question, check with our office before scheduling an appointment with a specialist to be sure he/she is in our referral network.
If you experience a situation where you need emergency care, please call our office at 978-948-3902 and we will expedite the processing of your referral. ^ top

Prescription Refills

We are happy to refill your prescriptions, if it is a medication prescribed for you by one of our Clinicians. Seventy-two (72) business hours notification is required for a prescription refill request.
If you need a prescription refilled, Email -- refills@cornerstone-ma.com.

• Step 1: State your name, address, and telephone number.

• Step 2: State the name of the medication, the dosage, how often it is dispensed and the quantity.

• Step 3: State the name of the pharmacy and the pharmacy telephone number.

Prescription refills that cannot be called or faxed to a local pharmacy will need to be picked up at our office (for instance, controlled substance medications require a written prescription be delivered to the pharmacy).

Insurance

We participate with many different insurance companies, each of which has its own rules and requirements. When you come in for your appointment, please have your insurance card ready and inform us of any changes in your coverage. Although we are always happy to assist you with insurance issues, it is important that you fully understand your policy and your responsibilities within it.

Emergencies

If you have an emergency, you should go directly to your nearest Emergency Room. If you are not sure or would like to speak to a Cornerstone provider about an urgent medical problem, please call us at
